Luther Haven Benefits:

Full time Employees:
Life - Waiting period of 1st of the month following 60 days. $15,000 policy at no cost to the employee.
Health – Waiting period of 1st of the month following 60 days. Luther Haven pays employee only premium at 100% and additional affordable dependent and family plans.
Dental – Waiting period of 1st of the month following 60 days. Voluntary plan – no employer contribution.
Cafeteria Benefit Plan – Flex Benefit that runs through payroll to set aside pre-tax dollars for either Medical/Health Care expenses or Dependent/Child Care expenses. Waiting period of 1st of the month following 60 days. Medical is a $2000 max and Dependent Care is $5000.

Fulltime and Partime:

Tax Sheltered Annuity Plan - Eligible from date of hire. Voluntary plan. Luther Haven will make a contribution to the employee Annuity Plan after they have worked 1000 hours in one year for the ensuing time of their employment.

Paid Time Off (PTO) - used for vacation time or sick leave.  PTO will accrue from date of hire, but is not available to be used until the successful completion of 90 days of employment. 

Extended Ill Time (EIT) - EIT will accrue from date of hire, but is not available to be used until the successful completion of 90 days of employment. 

Holidays – 7 holidays granted for Holiday pay: New Year’s Day, Easter, Memorial Day, Fourth of July, Labor Day, Thanksgiving Day, and Christmas Day.
